By signing this form, you acknowledge that you have voluntarily chosen to participate in physical training, kickboxing, sparring, bootcamps, personal training, seminars, and any other activities conducted by Jordan Waring, trading as WAR Kickboxing. You understand that participation in these activities involves inherent risks, including but not limited to: muscle strain, impact injury, falls, or other physical harm.
You confirm that you are voluntarily participating in these activities with full knowledge of the potential risks. You accept all responsibility for any injury, illness, loss, or damage you may suffer as a result of your participation, and you fully release and discharge WAR Kickboxing, Jordan Waring, and any staff, coaches, assistants, or agents from any and all liability, claims, demands, or causes of action arising from your participation, whether caused by negligence or otherwise.
You confirm that you are physically fit and have no known medical condition that would prevent safe participation. You agree to inform WAR Kickboxing of any medical condition, injury, or limitation prior to participating. You authorise first aid or medical treatment to be administered in the event of injury, illness, or emergency, and understand that you are solely responsible for any resulting costs.
You consent to the use of photography, video recordings, or digital media taken during sessions, classes, or events for marketing, educational, and promotional purposes, without compensation. These materials remain the property of WAR Kickboxing.
For participants under 18: A parent or guardian must read and sign this form, confirming their understanding and consent for the child’s participation and accepting all associated risks on the minor’s behalf.
